Abstract: 

Background & Objective: Patients have the right to be informed about their own health condition including Bad News and sometimes, medical staff has no choice but to give the patients such information that makes them feel Bad in patients and therefore, evaluate as "Bad News".Unfortunately such kind of News is enormously found in medical environment, for medical staff, patients and their companies. The responsibility of telling patients the Bad News is one of the hardest tasks in medical career. Patients and their companies’ illogical expectation add to its difficulty while in this situation medical staff is also responsible for calming others.Conclusion: the difficult part of telling Bad News is due to physicians’ previous lack of skill, even the ones with a proven track record, for facing the situation of transferring Bad News and further management and consequences which may end in bringing it out of control and causes patients’ and their companies’ mistrust of staff or hospital environment. This ends in discontinue of future and useful necessary plans. This situation causes a faulty circle of unwanted events which are against the medical affairs. So a thoughtful Bad News transfer to patients and their companies may cause them a relief and makes the control of present and future consequences easier. This important factor will be gained by practicing and applying the smart strategy of SPIKES by the professional physician.

In medical practice, delivering Bad News to patients is a complicated issue. Clinicians sometimes are faced with the dilemma of delivering Bad News to a patient: to tell or not to tell the truth. In this context, the religion of patients is an important factor. Some people, such as Muslims, believe in life after death and Judgment Day. Accordingly, avoiding disclosure of the pertinent information to such patients is a clear violation of patients' rights because it denies them the opportunity to do penance and seek God's forgiveness for their sins. Abstract: 

Background: The manner in which healthcare professionals deliver Bad News affects the way it is received, interpreted, understood, and dealt with. Despite the fact that clinicians are responsible for breaking Bad News, it has been shown that they lack skills necessary to perform this task.Objectives: The purpose of this study was to explore Iranian mothers’ experiences to receive Bad News about their children cancer and to summarize suggestions for improving delivering Bad News by healthcare providers.Materials and Methods: A qualitative approach using content analysis was adopted. Semi-structured interviews were conducted with 14 mothers from two pediatric hospitals in Iran.Results: Five major categories emerged from the data analysis, including dumping information, shock and upset, emotional work, burden of delivering Bad News to the family members, and a room for multidisciplinary approach. Conclusions: Effective communication of healthcare team with mothers is required during breaking Bad News. Using multidisciplinary approaches to prevent harmful reactions and providing appropriate support are recommended.

Abstract: 

Breaking Bad News to the patients does not back to a long history and is a controversial issue between patients and physicians.Many physicians are reluctant to breaking Bad News to patients and this is not desirable for most patients. For example, in Northern European countries and United States, most physicians usually break Bad News to the patients, while in Southern and Eastern European countries or many Asian countries they would not do so. In Iran, physicians prefer to break Bad News to patient's family rather than the patient. Cultural differences also influence people's viewpoints about breaking Bad News. In Western countries, most people agree with breaking Bad News to patients while it is not common in the other populations. Nowadays, the dominant view in the most countries is that it is the duty of the physicians to break Bad News to patients. Some advantages of breaking Bad News to patients including strengthening the trust between physician and patient, preventing non- maleficence, increasing patients satisfaction and reducing legal action against the doctors There are some exceptions to breaking Bad News the most important is serious psychological damage to the patient. Quality and quantity of information that should be released depends on situation of each patient.Breaking Bad News needs specific communication skills and physicians must be trained for this purpose.
